Do you really think that 5yo is on chatroulette alone? maybe the dad (or older brother) isn't letting him see the screen until it's clean. Maybe the webcam isn't on the monitor, but is just pointed at the kid.
It's obviously a setup of some kind, since the person taking that snapshot is the "kid"
